The Journey of a Well Nurse: Assess Your Wellness Part 2

Print Friendly and PDF

nurse wellness journeyA successful wellness journey most often begins with this first step— a self-assessment of surrounding circumstances, or in other words, locating your “here” so that you are able to map your self-care journey to “there”, which is your final destination or vision of where you want to be.

Take this brief wellness assessment by following the steps below:

1.  To the left of each life category, rate your level of satisfaction on a scale of 0-10 in each area, with 10 being 100% totally satisfied.

Rate Satisfaction
0-10, 10 = 100%
Life Category

Rate Coping 0-10,
10 = Perfectly

  Environment (work)
  Finances
  Nutrition
  Physical Activity
  Relationships (home)
  Relationships (spiritual)
  Relationships (work)
  Sleep
  Thoughts/attitudes/self-talk
  Time

2.  Next, assess how you are coping with the categories that you rated satisfaction at a 5 or below by writing down in the right column a number that represents how well you feel you are coping with that life category on a scale of 0 to 10, 10 being “perfectly”.

The life categories that you are not highly or satisfactorily satisfied with and that also have a low rate of coping next to them will be the areas where you will have the most opportunity to make positive changes. You’ve just discovered your “here”. Congratulations!

In Part 3 of the Well Nurse series you will be able to choose your destination—your vision of where you want to be—with any or all of the life categories you are not currently satisfied with. You will be able to draw your map by choosing small SMART goals that will take you to where you want to be—“there”!
